Why should parents preserve umbilical blood?
A series questions can arise in pregnancy which is, why should parents preserve the umbilical cord blood? It is expensive; it is against the ethical groups and many other factors. Few scientists think that there can be complication after the transplantation. The patient may be affected with the germ, Graft Versus Host Disease (commonly known as GVHD). The negative use of cord blood cell may vary from mild to severe. The cord blood collected does not cater to an adult as abundant amount of the blood cannot be collected through the umbilical cord. Further, it takes a little longer time for the new cells to be generated.
Despite of all facts, it is always advisable to store the umbilical cord blood. This blood is rich in stem cells. The undifferentiated cells which can differentiate into specialized cell types are called stem cells. Stem cells can be broadly divided into two parts, embryo stem cells and adult stem cells. Further, there are majorly three types of stem cells; they are Hematopoietic stem cells, Mesenchymal stem cells and Endothelial stem cells. In common language the stem cells are also known as the building blocks of the body. This umbilical cord blood cell can cure more than 70 types of fatal diseases. There has been more than 4000 umbilical cord blood transplant in the last ten years. Previously it was believed that bone marrow and the peripheral blood are the only two sources of stem cells. But this recent invention has proved that the blood in the umbilical cord is rich in stem cells to. The stem cells helps to produce white blood cells, red blood cells and platelets too. The moment the baby is born, the cord blood used to be thrown away. But now expecting parents need to inform the medical centers on the 34th week of the pregnancy. The health centers make all the arrangements to collect the blood. The blood needs to be collected within 10 minutes from the delivery. At least 75 ml of blood from the umbilical cord is required. Once the blood is collected after testing it, several chemical are added to preserve it. Further, the blood may be stored in a public or a private bank, depending on the parent’s choice. By preserving the cord blood parent assure a safe life for their kids.
Cord blood stem cell transplantation was first used for a 5 years old kid. In the year 1988, this child was affected by Fanconi’s anemia. It is when the child’s sibling’s cord blood was used. Since then the use of the cord blood cell has been started. Apart from providing medical benefits stem cells helps in therapeutic cloning and regenerative medicines. It acts as a medicine for diseases like Parkinson’s diseases, Schizophrenia, Alzheimer’s diseases, Cancer, Spinal Cord injury, diabetes and many more. With the help of stem cells few organs and limbs can be developed in labs and then used for transplantation. Many drugs are been tested on the stem cells and not on animals or human testers. Stem cells can detect the power of the drugs and assist to detect the effect of the same of humans. Stem cells have proved fatal in the study of abnormal birth.

In the recent past there has been a proliferation of family cord blood banks (for-profit companies that facilitate the collection and storage of umbilical cord blood for personal or family use), which are been rigorously advertised in public. This seeks the attention of the physicians too. Few survey conducting organisations had conducted few surveys to know the view of the parent and doctors regarding Cord blood banking. The survey was conducted mainly in USA and Canada and the doctors were all identified through IBMTR. The completed survey was received from 94/130 (72%) eligible respondents from 58 centres.
Around 51% of the respondents were clinical directors. Out of all the experiments 7 autologous cord blood transplants were performed at 6 medical centres. After all the experiments the families were asked if they would recommend family cord blood banking for their child with a healthy 3 year-old sibling. Te results came out as: 25% neutral, 47% recommended against, and 28% strongly recommended against. When parents were of mixed ethnicity, only 11% recommended banking (with 32% neutral, 42% recommending against, and 15% strongly recommending against). Thus we can understand that there is mixed reaction of parent in storing and preserving the cord blood for their child.
Few cross sectional surveys were emailed to pediatric hematopoietic cell transplantation physicians in the United States and Canada to know their view on cord blood banking. As a result of the survey it was studied that 55% of the population would choose autologous cord blood for neuroblastoma or for aplastic anemia. They preferred this in case there was no sibling donor available. The North European descent would generally not avail cord blood banking if they had at least one healthy sibling. Parent from other ethnicity would highly recommend cord blood banking for their child’s future.
In the absence of a known indication in the recipient, certain transplants using the cord blood stored in the private bank has already been performed. It all depends on the disease and the other alternative availability of the stem cells. If there is no other source to use the stem cells and the disease requires stem cells then cord blood from the banks are used as there are no other alternatives. Certain Hematopoietic cell transplantation physicians also support the private cord blood banking in the absence of an identified recipient.
However in the last one year the blood banks to store cord blood has increased in number. It has been seen that the cord blood banking falls under two camps while experiencing growth. One if the flat line growth which has less than 3% annual growth the other is the substantively building revenue which has more than 17% or more annual growth.
Still today cord blood is been used extensively to save lives of million. Stem cells from the cord blood have been used in more than 14000 transplants worldwide. Pediatricians, family physicians, obstetricians, nurse midwives, and other professionals who work with families should educate parents about the medical community's consensus view on this issue."
Dr. Lubin’s view on use and preservation of umbilical cord blood cells is that: “Physicians have a responsibility to suggest sibling cord blood banking in cases where a current child in the family could potentially benefit from a cord blood transplant, a view that is consistent with AAP's current policy statement. Where programs exist, donation of cord blood to public banks should be encouraged”.
Dr. Charles J. Lockwood who is the head of the department of obstetrics, gynecology, and reproductive sciences at Yale University, New Haven says that: "The only group to whom [private cord blood banking] could be recommended without hesitation are families with prior children affected by genetic or acquired conditions amenable to allogeneic bone marrow transplantation".

Special cryobags are used to store the cord blood. These bags are specially compartmentalized. There are three segments in the bag and the portions include 20 ml and 5 ml. Extra protection is given as the bags are over wrapped. The cells of the blood need to be stored for long term as it is the main part of the blood. Hence the technique which is used to preserve the cord blood is controlled rate freezing. Once the blood is taken in the specialized bag it is put in a protective cassette and placed in liquid nitrogen. This keeps the cord blood frozen. The preservation method is monitored. The temperature is mainly monitored. Monitoring of the blood is required as the blood is used for critical diseases. Till the stem cell is stored parent have no control over the usage of the blood. Once it is stored one cannot use the same without the parent’s consent. It is believed that the stem cell can be stored for decades.
